All forms must be submitted on or before the 15th of the month prior to your child's birthday month (e.g., your child's birthday is May 2, form must be submitted by April 15)
$10 for one birthday message, each additional birthday message in your immediate family (siblings only) is $5 (e.g., 2 messages for $15, 3 messages for $20)
